Kanboard - w/ one of it's theme plugins to restyle in github's style. I recently found /r/selfhosted and am losing control to the machines. Github also has a kanban like system I think but I wanted something I could fiddle with the code on.

WeKan also recommended as a decent option but not sold on the UI/simplicity as much as I like kanboard's.
