All I can say is try and be a pain in the ass everywhere you can. Keep faxing in your info. @Tripsa has a bunch of different numbers you can try faxing. Call payments, call amazon. Try the jeff@amazon.com email address too. I got a reply from that one eventually.

 

you could call the general amazon phone support and they can at least open tickets for you or something. It is worth a shot on top of everything else.
