You are penalized if you have any rejections at all, in that you are unable to complete any hits that require a 99% or better approval rating. Once you receive your first rejection you are permanently barred from completing any of these HITs.

The next tier of penalty occurs if you slip below 99% approval, that is, if you have more than 1 rejection for every 100 HITs. This is the threshold almost everyone sticks to; a bad requester can torpedo 100% with a bad rejection, but only working on decent tasks from reasonable requesters should make it easy to keep rejections within the 1/1,000 to 1/10,000 range.

Amazon can choose to suspend or terminate an account at any time, regardless of approval rate or total number of rejections. Obvious things to avoid include working on tasks that obviously violate the Mturk terms of service, or completing tasks in a way that violates terms of service (being dishonest, using a bot to complete tasks, behaving inappropriately or in a harassing manner). If Amazon decides to terminate an account for cause, any balance in the Mturk account is forfeit.

Assume any account suspension will be a termination, so don't leave more money in your account balance than you feel comfortable losing.
