    Also correct me if I'm wrong but I think it's possible when you refresh with accept next checked it will grab another every time in your queue, unless you only refresh the frame. Check your queue to make sure it is empty.

    I had that weird one expire on me then I started getting those errors too. I switched to that sentence study, then when I came back it was all gucci!

    Correct, most of the time. The exception would be when a HIT doesn't have its own iframe (external submit button enabled, URL bar reads mturk.com/submit). You'll get the "Do you want to submit this data blah blah" message again on those HITs.
