Pros:
The link DOES work - you have to right click and open in a new tab. I got an error when simply left clicking.
The survey itself is one page, including 2 demographics questions. Some bubbles, but really, it's very short.

Cons:
The wording is not the best, but you can understand what they're trying to to ask.
