This error is preventing me from accepting any surveys. I live in the US and I don't use a VPN. Has this happen to anyone else, if so how do I fix it.

Maybe you did the hit before or someone on the same ip has.

Are you sure that you haven't returned that survey before? Either that or your ISP has you on a VLAN with another turker.
