Hourly rate is of all requester's reviews or is it a weighted average with the last X number of reviews given more weight? Because I think it would be more accurate to use the latter formula.

 

It's the former, but the latter information is also available on the requester's TV page in the form of the graph at the top. For more recent data I usually look at the data for the hit itself rather than the requester.
